Referring a Patient

Hospice of Acadiana receives referrals from many sources: from the patients themselves, their family members or friends, or other healthcare providers including those in physician's offices, nursing homes, hospitals, assisted living facilities, home health companies and other providers.  Once Hospice of Acadiana receives the referral of a patient, we contact the patient's physician to see whether the the patient is eligible for hospice care with a terminal prognosis.  We then ask the physician whether he or she would like to remain the attending physician.  After working with the physician, we then contact the patient and/or family, or caregiver to set up an appointment to meet with them to explain our services as well as the options for the course of care of their loved one.  Hospice of Acadiana's Staff may also ask the physician's office for the following information:

 

  • Admitting diagnosis and prognosis

  • Current medical findings

  • Orders for medications and treatments

  • Patient and family understanding of disease and prognosis

  • History and Physical

  • Order for Hospice Care

 

At the time of admission, the hospice interdisciplinary team will complete a comprehensive assessment of the patient/family needs. Our hospice interdisciplinary team will then develop a plan of care specific to the individual needs of each patient/family.
